Donepezil
Acetylcholinesterase inhibiting drugs are used in the treatment of Alzheimer's disease, speciality for mild to moderate disease. It is a reversible inhibitor of acetylcholinesterase.
INDICATIONS: Dementia.
CONTRAINDICATIONS: Pregnancy, lactation, hepatic impairment, cardiac disease, chronic asthma.
SAFETY PROFILE : Sick sinus syndrome, supraventricular conduction abnormalities. Susceptibility to peptic ulcers, asthma, COPD.
ADVERSE EFFECTS: Occasionally headache, bradycardia, AV block, nausea, vomiting, fatigue, convulsions, diarrhoea, anorexia, gastric duodenal ulcer, GI haemorrhage, urinary incontinence, possibly bladder outflow obstruction and hepatitis.
DRUG INTERACTIONS : Antiarrhythmics (Procainamide, quinidine, propafenone), Anti-bacterials (Aminoglycosides, clindamycin, colistin), and lithium, antagonise effect of neostigmine and pyridostigmine. Antimalarials (Chloroquine., hydroxy chloroquine) increase symptoms of myasthenia gravis.
DOSAGE: 5mg once daily at bed time if necessary increase to 10mg daily after one month, maximum dose 10mg daily in single dose at bed time.
