Nimodipine
It is a calcium "channel blocker. It enters the brain and it acts by blocking the entry of extracellular calcium in neurones. Being highly lipophilic nimodipine has greater action on the cerebral vessels.
INDICATIONS: Subarachnoid haemorrhage, ischaemic neurological deficits senile dementia,
CONTRAINDICATIONS : History of allergy, hypersensitivity, Hypotension.
SAFETY PROFILE: Patients with hypotension, patients with impaired renal and hepatic functions, patients with cerebral oedema, patients with raised intra cranial pressure, monitor pulse and BP during IV therapy.
ADVEKSE EFFECTS: Epigastric distress, fall in blood pressure, flushing, nausea, disseminated in-travascular coagulation, hepatitis, haemorrhagic in¬farction, tachycardia, Gl symptoms fatigue.
DRUG INTERACTIONS : Phenytoin and caffeine : delayed metabolism of these substances., increase the toxicity of these drug and other calcium channel blockers: Increased effect.
DOSAGE: 60mg. 4 hourly Starting within 4 days of subarachnoid haemorrhage.
IV. : Infusion via central catheter 1 mg per hour initially, increased after 2 hours to 2mg/hour unstable patients or <50kg. weight 500 mcg./hour initially. Treatment should be continued for at least 5 days.
