INDINAVIR SULPHATE : It is an inhibitor of the enzyme human immunodeficiency virus protease which is required for the proteolytic cleavage of the viral polyprotein precursors into the individual functional proteins found in infectious HIV.

Indications: Treatment of HIV infection in combination with antiretroviral agents.

Dosage: The recommonded dosage of indinavir is 800mg (two 400mg capsules) orally every 8 hours. It should be administered without food but with water 1 hour before or 2 hours after a meal. To ensure adequate hydration, patient should drink atleast 1.5 litres of liquids in 24 hours.

Contraindications: Hypersensitivity. Avoid concurrent use with terfenadine, cisapride, astemizole, triazolam, midazolam, or ergot derivatives.

Special precautions: Hepatic impairment, Monitor blood glucose levels since hyperglycaemia may occur, pregnancy, lactation.

Side effects:- Nausea, vomiting, diarrhoea, abdominal discomfort, dry mouth, taste distrubances; headache, dizziness, insomnia; myalgia, asthenia, paraesthesia; rash, pruritus, dry skin, hyperpigmentation, nephrolithiasis (may require interruption or discontinuation), dysuria, haematuria, crystalluria, proteinuria; elevated liver enzymes and bilirubin, hepatitis; blood disorders including neutropenia, haemolytic anaemia; hyperglycaemia.

Drug Interactions: HMG-CoA reductase inhibitors (atorvastatin, cerivastain, lovastatin, etc.), Rifampin, Rifabutin, Ketoconazole, Grapefruit juice, Nevirapine.
