TERBINAFINE: It is a synthetic antifungal compund and is an lllylamine derivative. It acts by inhibition of squalene epoxidase in the fungal cell membrane.
Indications: Fungal infections of the skin caused by damatophytes like Trichophyton spp,. Microsporum spp,. Epidermopyton spp,. Orally indicated in the treatment of onychomycosis caused by dermatophytes.
Dosage : Adults : 250mg for a duration of 2-6 weeks in Tinea pedia, 2-4 weeks in Tinea corporis/curuis, 2-4 weeks in Tinea capitis, 6-12 weeks in candidiasis, weeks in Tinea capitis, 6-12 weeks in onychromycosis.
Children : 2 yrs and old : Upto 20kg bodyweight and above): 62.5 mg once a day; 20 to 40 kg body weight 125 mg once a day;> 40kg bodyweight; 250 mg once a day.
Contraindications: Hypersensitivity to terbinafine.
Special Precautions: Pre-exisiting liver or renal impairment, irregular use or premature discontinuation carries the risk of recurrence of infection. Elderly patients, pregnancy, lactation.
Side-effects: Gastrointestinal symptoms, skin reactions taste disturbace, hepatobiliary dysfunction, heanatological disorders.
Drug Interactions: Negligible potential for inhibiting or inducing the clearance of drugs that are metabolised via cytochrome system e.g. cyclosporin, tolbutamide or oral cantraceptives. Drug that induce or inhibit microsomal enzymes may interfere with terbinafine metabolism. Terbinafine decreases caffeine clearance.
