Amrinone
Amrinone is a Cardiac inotrope. It is used for treating congestive heart failure. It is studied for useing before coronary artery bypass surgery. Amrinone injection is a phosphodiesterase enzyme inhibitor that maximizes force of contraction of heart. This drug increases cardiac contractility, vasodilator.
It’s a positive inotropic agent with vasodilatory activity, different in structure and mode of action from either catecholamines or digitalis glycosides. While using, Amrinone should be diluted with normal saline or 1/2 normal saline. Pulse, blood pressure and EKG of the patient should be constantly monitored when amrinone is given.
Technical detail
INDICATIONS: Congestive heart failure.
DOSAGE: Adults: Initial loading dose: 0.75mg/kg as slow IV over 2-3 minutes, followed by maintenance infusion of 5-10 mcg/kg/min. the loading dose may be reported after 30 minutes, if required. Total max daily dose 10mg/kg.
Children: not recommended.
CONTRAINDICATIONS: Hypersensitivity.
SIDE-EFFECTS: Cardiac arrhythmia, hepatoxicity, GI disturbances, myositis, nail discoloration, nausea, hypersensitivity, vomiting, thrombocytopenia, chest pain, hypotension,
DRUG INTERACTIONS: Exaggerated hypotension reported with concurrent usage of disopyramide and amrinone.
OTHERS: Chemical interaction with precipitation occurs with amrinone and dextrose or frusemide. These drug mixing should be avoided.
