SOMATOSTATIN: It is a synthetic cyclic tetradecapeptide having the structure of the hypothalamic hormone that inhibits the release of growth hormone.
Indications: Acute severe bleeding oesophageal varices, acute upper GI bleeding resulting from gastric/duodenal ulcers and haemorrhagic gastritis. Intestinal and pancreatic fistulae. Hypersecretory endocrine tumours of the intestinal tract.
Dosage: Acute bleeding oesophageal varices: IV bolus of 250 meg over 1 minute, followed by 6 mg/ 24 Hr continuous infusion for 5 days. In case of interruption new IV bolus 250 meg should be given. Upper GI bleeding from gastric/duodenal ulcers: IV bolus of 250 mcg followed by 3 mg 12 hourly continuous infusion for 2-5 days.
Intestinal & pancreatic fistulae: IV bolus not required. Infusion 6 mgday may be required up to 14 days. After closure of fistulae infusion should be at half-dosage for the next 48 hrs.
Contraindications: Hypersensitivity.
Special precautions: Blood glucose levels must be monitored, particularly in diabetics'. In patients with fistula taper the dose after closure of the fistulae.
Side-effects: Abdominal discomfort, flushing, nausea and bradycardia, stetorrhea, dyspepsia, hypo-chlorhydria, cholelithiasis, mild watery diarra.
