Baclofen

Baclofen is an antispastic agent and a muscle relaxer, which is used to treat muscle symptoms.It is also used for some other purposes. It's vey efffective in the muscle symptoms caused by multiple sclerosis like pain, spasm and stiffness. It has been proved in some studies that Baclofen is also effective in alcohol withdrawl syndromes.

To get rid of the side effects of baclofen, it shoud be used carefully. It may impair the thinking of a person. If a person is doing some work that requires high alertness, then the use of baclofen should be avoided. Alcohol increases the side effects of baclofe. Therefore, alcohol should be strictly avoided while using baclofen. Baclofen may reduce muscle tone. The use of baclofen should not be stopped suddenly. It may also cause some side effects.

Technical detail

It is centrally acting muscle relaxant. It is an antiseptic agent. It inhibit mono-synapticans polysynaptic reflex transmission at spinal level. Its effect due to stimulation of GABA Incription receptors.

INDICATIONS:

1. Symptomatic management of acute muscle spasm eg.torticolism tearing of ligaments, fibrositis.
2. Anxiety and tension associated with increased muscle tone.
3. Hyper-reflexia.
4. Dystonia.
5. Hiccup.

CONTRAINDICATIONS: Hypersnsitivity to baclofen.

SAFETY PROFILE: Peptic ulcer, psychiatric disorder depression, H/o epilepsy/convulsion, Hepatic & renal fun. Impairment, Hypertonic bladder.

ADVERSE EFFECTS: Drowsiness, dizziness, weakness, nausea, confusion, euphoria, hypotension, hallucination, depression, headache tremor, insomnia, visual disturbance.

Muscular - Muscular hypotonia, respiratory depression, coma, convulsion.

DRUG INTERACTIONS:

1. CNS depressant drug and alcohol, potentiate the effect of Baclofen.
2. Tricyclic antidepressant, lithium increases hyperkinetic symptoms.
3. Abrupt withdrawal of baclofen should be avoided.

DOSAGE: Adults: Initially 5 mg. t.i.d. orally. Total daily dose increased by 15 mg. every fourth day till 20 mg. t.i.d. or desired therapeutic effect observed. CHILDREN: 0.75-2.5 mg/kg/day. Maintenance dose: 10 - 20 mg. 12 months-2 year 30 - 60 mg. 6-12 yrs.

INTRATHECAL DOSE: Test dose-25 to 50mg./day. Increased by 25 mg. every 24 hours. Till total dose is 100 mg. In tetanus - Continuous iv.Infusion is given.
