Duloxetine

It is an antidepressant, serotonin/ Norepinephrine reuptake inhibitor drug. It has no significant activity for muscarinic cholinergic, H1-histaminergic or alpha2 adrenergic receptors. Duloxetine does not possess MAO inhibitory activity.

INDICATIONS: Treatment of major depressive disorders in adults.

CONTRAINDICATIONS: Hypersensitivity, concomitant use of MAO inhibitors, Uncontrolled narrow angle glaucoma.

SAFETY PROFILE: Caution required in Urinary hesitancy, CNS depression, hepatic impairment. May cause or exacerbate sexual dysfunction, controlled narrow angle glaucoma, Prescribe to a pregnant woman it potential benefit justifies the potential risk to the fetus.

WARNING: Sucidal risk: The possibility of a suicide attempt is inherent in major depression and may persist until remission occurs. Monitor for worsening of depression or suicidality, especially during initiation of therapy or with dose increases or decreases. Worsening depression and severe abrupt suicidality that are not part of the presenting symptoms may require discontinuation or modification of drug therapy.

ADVERSE EFFECTS: Insomnia, Constipation, dizziness, fatigue, pruritus, rash, blurred vision, tremor, decrease lipido, increased sweating, nausea, appetite decreased.

DRUG INTERACTIONS: CYP1A2 inducers Aminoglutethimide, Carbamazepine, phenobarbital and rifampin may decrease the level/effects of duloxetine.

CYP1A2 inhibitors (For e.g amiodarone, ciprofloxacin, fluvoxamine) and CYP2D6 inhibitors e.g. (paroxetine, fluoxetine, Quinidine, ritonavir may increase the levels/effects of duloxetine. Concurrant use of duloxetine with buspirone may cause serotonin syndrome.)

Duloxetine may increase desipramine levels. Concurrent use may results in toxicity, weakness, hyper-reflexia and incordination have been observed with sumatripton and SSRIs (and/on SNRIs).

DOSAGE : Adults : Oral : Treatment of major depressive disorder : Initial : 40-60 mg/day (dose may be divided , i.e. 20 or 30 mg twice daily or given as a single daily dose of 60mg) Maximum dose : 60mg/ day.

Management of diabetic neuropathy (unlabeled use) : 60-120mg/day (60mg once or twice daily)

Management of Chronic pain syndromes (unlabeled use): 60mg once daily.

Management of stress incontinence (unlabeled use: 40mg twice daily. Capsules should be swallowed whole. Do not break, open or crush.
