RABIES VACCINE: -RpfSies vaccines which are used for active immunisation against rabies may be used as part of post exposure treatment. It may also be used as preexposure prophylaxis against rabies in high risk persons like dog handlers etc. Rabies vaccines may be :
- Purified chick embryo cell rabies vaccines.
- Inactivated Rabies vaccine. iii. Human Diploid Cell Rabies Vaccines.
Adverse Effects: Pain, reddening and swelling at injection site, swollen lymph nodes, joint pains and G.I. complaints.
Contraindication For immunization after exposure none. For prophyladis postpone in pregnancy. Hyparesensitivity to constitutions of vaccine.
Spelcal Precautions: Pateints receiving irnmuno-suppressive therapy. Indications For immunization against rabies after exposure and for prophylactic vaccination against rabies before exposure.
Dosage : Vaccine should be given intramuscular in the deltoid region only. (a) pre-exposure (Before exposure): 3 dose i.m. injections on day 0, day 7, day 28. A booster dose after 1 year and 1 dose after every 5 years. In case of subsequent exposure, only 2 doses af DO and D3 provide protection, if proper previous vaccination status is available. (b)Post-exposure : After exposure start immediately a full course of treatment for both adults and children consists of 5 injections on days 0 (day of exposure), 3, 7, 14 and 30. A booster dose on D90 is optional.
PURIFIED CHICK-EMBRYO CELL RABIES vaccine:
VACCINE: Vial: 1Dose of freeze dried substance, containing = 2.5 i.u. inactivated rabies antigen (virus multiplied in chicken fibrobiast cell cultures). Stabilizer (1Dose) TEN -haemacel & 0.1% Gluta-mate q.s.
Ampoule: Solvent-sterile water for injection. The antibiotics (Neomycin, chlortetracycline, Ampho-tericin B) added during cell and virus propagation are removed to the greatest possible extent by purifiĀcation steps and cannot be detected with currently used methods in final vaccine product.
Indications: For immunization against rabies after exposure. For prophylactic vaccination against rabies before exposure.
Dosage: After exposure; start immediately . A full adults & children) One dose on each of days 0,3,7,14,30 and 90. 6th dose on day 90 is optional. If animal is suspected to be rabid then shouldin addition receive rabies immunoglobin (human origin) at a dose of 1x40 i.u. per kg of body weight. The injection should be given together with the first dose of vaccine, but at different sites of the body. Before exposure: A full course of prophylactic treatment consists of one dose of the vaccine on each of days 0,28 and 56, or in more urgent cases on day 0, 7 and 21.
Contra-indications: For immunization after sure there are no contraindications. Before exposure: Postpone in pregnancy. Hyper-sensitivity to constituents of vaccine.
Special Precautions: Patients undergoing immu nosuppressive therapy.
